	*Health Contract with Cyprus Life Hospital*

HEALTH CONTRACT

This health contract is the annual agreement on behalf of the British Residents Society and Cyprus Life Hospital under the following conditions.

1. Each contract is an agreement solely between the Cyprus Life Hospital and the individual named in the contract.

2. The contract is for use only by the individual named therein. It is not transferable under any circumstances.

3. The contract is only available to fully paid up members of the British Residents Society. Any benefits accruing there from can only be obtained on production of a current BRS membership card. The Cyprus Life Hospital reserves the right to withhold any benefits if this membership card cannot be produced when requested.

4. The contract will be valid for a period of 12 months from the date of its validation.

5. The Cyprus Life Hospital agrees to accept in to this scheme any fully paid up member of the British Residents Society irrespective of age or medical condition. The costs and benefits of the scheme will be the same for all such members.

6. The contract is only applicable to the services provided by the Cyprus Life Hospital. Currently these relate to the following areas of medical practice:- Obstetric and gynaecology, paediatric, medical outpatient, general surgery, *plastic surgery, ophthalmology, radiology, and diagnostic department, urology, cardiology, dental surgery, orthopaedic surgery, spinal surgery, ENT, neurology, neurosurgery, nephrology, dietician, dermatology, psychology, physiotherapy and rehabilitation, tomography, x-ray, scop0e, mammography, bone density, Doppler ultra-sound. Biochemistry, microbiology and pathology laboratories are also available, as are facilities for medicals for residency and work permit requirements.

*please note that as far as plastic surgery is concerned this contract covers only medical needs (e.g. burns, accidents etc.) It does not cover cosmetic needs, (facelifts etc.)

The Cyprus Life Hospital runs a very comprehensive and detailed

Programme for annual checkups. This service is not included in

this contract but any member wishing to use this service will be entitled

to a discount as set out below.

7. In circumstance where any facilities are not currently available at the Cyprus Life Hospital; and which may require attendance at health facilities outside the TRNC the Cyprus Life Hospital undertakes to provide advice and its best offices to assist any contract holder.

8. The scale of charges made for any services by the hospital will be those within the Cyprus Turkish Medical Association Price Guide ranges. (a copy of which can be made available on request.) less the relevant discount rate as set out below.

9. The Cyprus Life Hospital reserves the right to cancel any individual contract for any violation of its terms.

10. Cyprus Life Hospital offers four kinds of membership card. These are Regular Membership, Bronze Membership, Silver Membership and Gold Membership. Each membership card for fully paid up members of the British Residents Society supplies the following multiple benefits to each card holder.

Regular Membership

Price: GBP 50 Sterling annually.

Benefits : 20% discount on the medical examination, 10% discount on other facilities.

Bronze Membership

Price: GBP 200 Sterling annually.

Benefits: 20% discount on all facilities.

Silver Membership

Price: GBP 400 Sterling annually.

Benefits: 35% discount on all facilities.

Gold Membership

Price GBP 600 Sterling annually.

Benefits: 50% discount on all facilities.

In addition

a) Any members joining as a married couple or family is entitled to a 10% discount for each member on the relevant membership fee.

b) Any member wishing to avail him/her self of the annual check up service is entitled to a % discount on the fee for this service.
